  2. Once your credit/debit card payment as been completed you will be returned to our thank you page asking you to fill out your ordering information and birth information.
  3. Click the "Send" button to email this order form. As soon as we receive this information we will process your order promptly.

    When ordering reports, be sure to check the emailed or mailed option. Shipping fees ($8-$16) only apply to mailed reports; we will contact you with this amount, all mailed reports are spiral bound for ease of reading at no additional cost.
